JVA VOLLEYBALL INSURANCE WAIVER

The waiver at the following link should be filled out prior to tryouts.

Please select “Westmoreland Elite” from the drop down and use my name (Lindsay Lesofsky) as the club director and email LindsayL.WEVBC@gmail.com.

NOTE: You do NOT need a USAV Membership or USAV Medical Release form to tryouts for Westmoreland Elite. You only need to fill out the waiver above and tryout fee to tryout!

$50 non-refundable tryout fee
